Older Adults: Support HCP consumers with falls prevention, dementia care, and aging-in-place solutions

NDIS Participants: Empower clients with disabilities to achieve their goals

Children and Youth: Address developmental delays and school-based challenges

Your Impactful Role Will Include:

Conducting comprehensive functional assessments

Developing personalized intervention plans

Prescribing assistive tech and home modifications

Navigating NDIS, HCP, and CDM funding systems

Building relationships with clients, families, and care teams

We're Looking For Someone With:

? AHPRA registration

? Passion for client-centered, community-based care

? Excellent communication and problem-solving skills

? Current driver's license and reliable vehicle

? Working with Children Check (or willingness to obtain)
