On 1 January 2026 five health services (Kooweerup Regional Health Service, Peninsula Health, Alfred Health, Bass Coast Health, and Gippsland Southern Health Service) will come together to create Bayside Health. A newly merged health service with more than 22,000 employees caring for a community of over 1.2 million Victorians.
The new health service will provide people with the best possible care with services that support them throughout their lives. The merger will also strengthen our workforce, providing new training, education, and career opportunities. Subject to final approval on 1 January 2026, all employees from the five health services will transfer to Bayside Health.

What You Will Be Doing
The Community Care Occupational Therapist forms part of Peninsula Health's Health Independence Program and utilizes the Wagner Chronic Care Model, an evidence-based systems framework, to guide their practice. The role will form part of Community Care to provide additional capacity for our team to respond to the Better at Home Strategy. The Community Care Occupational Therapist is responsible for coordinating intensive short-term (3-6 months) outreach care and delivering allied health interventions within their clinical scope of practice to clients who have chronic health conditions and/or complex healthcare needs.
The role involves facilitating collaborative integrated care planning across acute and community-based services. The role of the community care occupational therapist supports both professionals and service users to navigate the health service system, ensuring responsive, patient-centric outcomes for all.

What You Need
We anticipate you will have:
Occupational therapist with current registration with the Australian Health Practitioner Regulation Agency (AHPRA) or relevant professional organization.
Demonstrated occupational therapist experience in providing comprehensive OT assessment in a home setting and provision of equipment needs.
Specialist experience and knowledge regarding chronic disease management.
Demonstrated experience working with at-risk and vulnerable people with complex needs.
Well-developed written, verbal, interpersonal, and computer literacy and communication skills.
Demonstrated experience in working both independently and as a part of a multidisciplinary team, with the ability to work effectively, supportively, and flexibly.
Demonstrated ability to manage multiple competing tasks and effectively prioritize.
Demonstrated commitment to professional development and evidence-based practice.
Demonstrated commitment to quality improvement activities.
Current Working with Children Check
Current Victorian Driver's License. Relevant postgraduate qualifications or working towards the same.
Demonstrated experience working with the Wagner Chronic Care Model.
Demonstrated ability to supervise occupational therapy students.
