Our client is a leading provider in Home Care and with the aged care reforms this is an exciting time to join their team as they expand and strengthen their Support at Home services. They are seeking a Home Care Coordinator to play a pivotal role in delivering high-quality, person-centred support across Home Care Packages (HCP), the new Support at Home program, and Commonwealth Home Support Programme (CHSP) clients. You will be supported by a Home Care Manager and working collaboratively be able to ensure the best care is provided to clients.
Key Responsibilities:
Coordinate, manage and optimise client packages
Carry out case management and conduct clinical assessments
Wound care, chronic disease and medication management
Support clients with financial management to ensure their needs are met
Monitor funding and expenditure to maintain services within budgets
Develop and review care plans that reflect client goals and clinical recommendations
Coordinate onboarding, reassessments, and case conferences
Work closely with intake, operations, and allied health professionals to ensure smooth service delivery
Complete commencement and exit documentation
Ensure overall compliance with the Aged Care Quality Standards
Maximise client funding and apply for supplements where appropriate
The successful candidate:
Experienced care coordinator who thrives on supporting people to live well at home
Previous aged care experience in a home/community setting
Current AHPRA registration as a RN or EN
Strong communication and organisational skills
Ability to manage competing priorities and a diverse caseload
Ability to work positively and collaboratively and add value
Working knowledge of aged care reforms and quality standards
A current driver's licence and reliable vehicle
NDIS Worker Screening Check and Police Check (or willingness to obtain)
Must have working rights within Australia
