Location: Tamworth NSW (Peel Street CBD base, community, and some work from home)

Classification: SCHADS Level 4.1to5.1
Salary: 88,099 to 100,783 + 12% super (annual review)
Travel reimbursement as per award. Structured supervision and paid training included.

Heart Worx provides Support Coordination, Psychosocial Recovery Coaching and direct support across the Tamworth region.
Were a small, lived-experience-led organization built on clear communication, realistic workloads and person-led practice.
Our team includes coordinators and recovery coaches who’ve personally navigated the NDIS, mental health systems or caring roles to we use that insight to keep our work grounded and human.
Our focus is helping people use their NDIS plans well and build steadier lives through practical, coordinated support.
This role center’s on planning, communication and advocacy, not direct care or therapy.
The Role
You’ll help participants use their NDIS plans effectively by: coordinating supports and communicating with providers, plan managers and clinicians tracking actions, budgets and outcomes through clear documentation maintaining realistic caseloads and structured workflows building your knowledge of NDIS systems and reporting through regular supervision
You’ll start with supported onboarding and shadowing, moving toward an independent caseload when ready.

What You’ll Bring
Strong writing and communication skills
Confidence with email, systems and detail management
A calm, reliable presence and sound judgement
Curiosity and willingness to learn complex systems
Interest in mental health, disability or community work
Some experience or volunteering in mental health, disability or community services is preferred e.g. support work, case work, employment services, youth or housing roles, or team leadership
Current NDIS Worker Screening, WWCC and Driver Licence (or ability to obtain)
