Your Role

As a Complex Support Needs Specialist, you will work closely with clients to develop tailored support plans, connect them with services and provide evidence-based interventions that promote recovery and independence. You will play a key role in helping clients navigate complex systems while fostering engagement, trust and positive change. This is a rewarding opportunity to work alongside a multidisciplinary team, providing specialist support coordination, therapeutic interventions and case management to help individuals build stability, strengthen wellbeing and achieve lasting positive outcomes.

Your Responsibility

Deliver specialist support coordination and case management through face-to-face appointments, telehealth and community outreach

Facilitate the Finding a Balance (FAB) psychoeducation program for clients unable to participate in a structured Day Program

Provide evidence-based therapeutic interventions in line with the AOD Model of Care

Complete intake assessments, risk screenings and client reviews

Develop, implement and monitor individual treatment and support plans

Support clients to identify and achieve personal goals

Build strong partnerships with external services to ensure coordinated care

Engage families, carers and significant others where appropriate

Maintain accurate and timely case notes and client records

Participate in supervision, case conferencing and team meetings

Contribute to service development, quality improvement initiatives and best practice delivery

Profile description:

You Will Need

A tertiary qualification in Social Work or Psychology

Experience supporting people impacted by AOD use, mental health concerns, homelessness, trauma and/or justice involvement

Experience working with clients with complex and intersecting support needs and confidence operating within safeguarding and risk-informed practice contexts

Strong communication, engagement and relationship-building skills

A commitment to ethical, person-centred and non-judgemental practice

Knowledge of community services, referral pathways and collaborative care models

The ability to work autonomously while contributing positively to a multidisciplinary team

Strong organisational skills and confidence using client management systems

An unrestricted NSW Driver Licence
