About us
Based Health Group was founded by two Occupational Therapists who believed allied health could be done a little differently, with high clinical standards, genuine team culture, and real care for the people we serve. About the role
This is a primarily clinical role for a Senior OT who leads from the front through the quality of their work, and who is ready to take on specialist work that others step back from.
You will carry a strong, complex caseload across NDIS, aged care, community home care, and Support at Home (SAH), spanning clinic rooms, private homes, residential aged care facilities, supported accommodation, custodial environments, and the community. The work covers high-needs physical presentations and complex psychosocial ones alike, including participants with AOD involvement, forensic histories, and highly complex social circumstances, including assessments and service delivery in and out of prisons.
This work requires presence. You will move between very different environments, often in the same week, and you will need the confidence and adaptability to hold your professional standard in all of them. You will bring a strong understanding of the NDIS system and its processes, alongside genuine knowledge of the ageing process and how it shapes function, risk, and support needs.
It is a hybrid role with real flexibility, variety, and autonomy, and a chance to lead. You will support and develop an excellent group of clinicians while setting the standard yourself in clinical quality, performance, and process. Independent, high-quality report writing is essential. The role is genuinely incentivised, with reward structured to match what you achieve.
Full time and part time arrangements are both available.
What you will do
Maintain a strong, complex caseload across NDIS, aged care, community home care, and SAH
Work across clinic, private homes, aged care facilities, supported accommodation, custodial settings, and the community
Manage high-needs physical presentations, complex manual handling, and significant functional decline
Deliver specialist services in forensic and justice settings, including assessments in and out of correctional facilities
Work with participants with AOD involvement, forensic histories, and complex social circumstances
Deliver Functional Capacity Assessments, Home and Living assessments, AT applications, equipment prescription, and home modification assessments to a high standard
Write independently, producing planner-ready reports without significant oversight
Engage confidently with mental health, ABI, trauma, AOD, and dual diagnoses
Help lead the clinical team through supervision and day-to-day mentoring
Drive clinical standards, documentation quality, and performance as we grow

Competent in AT and home modification assessments
Confident writing independently, with reports that are structured, clinically justified, and need minimal revision
