As a member of the Sunrise Health Service multidisciplinary team this position will promote culturally appropriate clinical care and healthy lifestyle messages to clients and community members.

The Chronic Disease Coordinator is to provide additional capacity to the remote health teams to deliver comprehensive, multi – disciplinary chronic disease clinical care to the people of the region.

Applicants  with a demonstrated ability to use personal initiative and  to actively participate at all levels with a high level of oral, written skills and with an emphasis on report writing, are encouraged to apply.

The position is located in Katherine and will include regular travel to remote communities in the Sunrise region. Clinicians applying for this position should have unconditional Australian Health Practitioner Regulation Agency (APHRA) registration.
