Case Worker (intervention)

Founded in 2004, Projets Autochtones du Québec’s (PAQ) mission is to provide shelter, emergency and culturally adapted social reinsertion services to First Nations, Inuit, and Metis peoples who are homeless or in situations of difficulty. PAQ is the only shelter accepting both Aboriginal men and women located in Montreal; this is what makes PAQ an essential resource to the community.
Responsibilities:
Under the supervision of the Coordinator, the case worker will:
1. Ensure a culturally-adapted intervention approach to respond to the needs and difficulties of the participants;
2. Provide holistic and comprehensive case management services to all clients including: intake assessment, goal setting, long-term case plan development, progress monitoring, advocacy and referrals.
3. Establish a confidence with the clients through active listening and positive interactions;
4. Work closely with clients to determine their needs and create an intervention plan for individuals based on those needs;
5. Be able to do group interventions and animation of cultural activities
6. Do accompaniments of clients to other resources as needed and facilitate their access to such resources;
7. Ensure regular follow-ups of all intervention plans
8. Communicate regularly with partner organizations to follow-up on referred clients;
9. Provide all required documentation in a timely manner, which may include client follow-up, outcome evaluation, client contact sheets, and evaluations.
10. Participate in staff meetings as arranged by the Coordinator;
11. Offer a flexibility with regards to the schedule in order to carry out various aspects of the job, such as, evening interventions and daytime one-on-one follow-up meetings and accompaniments;
12. Effectuate other tasks related to the position as determined by the Coordinator.
Work Status: Full-time, 35 hours per week
Schedule: Variable shift hours, Monday to Friday.
