The Opportunity: To better deliver on Amplar Health and Medibank’s prevention ambition, our Preventative Health Operations Squad delivers a range of funded programs including Health Concierge, CareComplete, and Health & Wellbeing Checks. A number of Senior Care Coordinator positions are now available which are central to providing valuable support to our leadership team in the Preventative Health Operations function which could include training, onboarding and participating continuous improvement activities.

Drawing on your subject matter expert knowledge and background in health, you will support your team of Care Coordinators and Program Clinicians to deliver safe and high-quality care coordination.  You will also be required to manage a clinical caseload – providing support to program participants who may have either had a recent hospital admission or require support for complex chronic illnesses.

About You: As either an AHPRA Registered Nurse or Allied Health professional we are looking for you to share our vision to shift the way healthcare is provided in Australia - bringing your passion around preventative health – ideally chronic disease management practices, and /or health behaviour change methodology. 

Previous experience across a range of leadership activities like training team members to achieve quality clinical outcomes, providing support with escalations, real time support/coaching and/or feedback to staff or conducting call audits/case reviews on team members will be key to your success.

Our service operates shifts between the hours of 08:00AM - 6:00PM (AEST) Monday to Friday.  This is a rare work from home opportunity.  As such you will need fast, reliable internet and appropriate workspace to ensure privacy whilst working.
