Where you'll be working Within the multi-disciplinary geriatric outreach team (GRACE) at Hornsby Ku-ring-gai Hospital (HKH),. What you'll be doing The position is required to provide a high-quality outreach service, providing care at home and aiming to avoid hospital admission for people who reside within the community in a private residence or within residential aged care facilities. The position works closely with the multi-disciplinary team, comprising of a Geriatrician & Medical Registrar, Nursing (CNC, CNS, RN), Allied Health (Physiotherapist) and Administration.

The GRACE service at HKH operates from Monday to Friday, 8am to 8 p.m. The successful applicant will be required to work rotating shifts of 8 or 10-hour duration. People of Aboriginal and/or Torres Strait Islander backgrounds are encouraged to apply.

Candidates will need to meet the following criteria: Registered with Authority to Practice with Australian Health Practitioner Regulation Agency. Minimum five years full-time post graduate experience with 3 years full-time equivalent experience in the specialty field. In addition, the applicant must have an approved post graduate qualification (minimum degree diploma or certificate level) or evidence of working towards Masters-level qualification.

Demonstrated clinical leadership skills and an ability to work collaboratively within a multidisciplinary framework with a demonstrated commitment to excellence in nursing practice. Demonstrated ability to undertake quality improvement and research activities that influence and enhance clinical practice and implement change. Current unencumbered NSW Driver's license.
