What You Will Be Doing  

Peninsula Health is seeking a full time Social Worker to join our team at Rosebud Hospital on a fixed term basis until mid-December 2025.  You will be responsible for completing psychosocial assessments, providing adjustment support and targeted interventions with both individuals and their families.  You will be required to have a sound knowledge of the psychosocial issues that people in our local community experience and will have experience in navigating relevant service systems.   You will have demonstrated experience in working collaboratively in multi-disciplinary teams to achieve patient goals and develop patient-centered discharge plans.  You will receive regular supervision and support from a dedicated onsite Senior Social Worker and work with a supportive team.

What You Need

We anticipate you will have:  

You will need to hold a Bachelor Degree or Qualifying Masters in Social Work and relevant post qualification work experience.  Post graduate studies are highly regarded and may be eligible for a higher education allowance. Grade 1 social workers with hospital experience who wish to transition to a Grade 2 social work role are encouraged to apply.

As a multi-site organisation you may be required to work across sites. You will have access to our extensive pool of fleet cars and will need a Victorian driver's license.
