About the Role: Join the Rehabilitation in the Home team and help patients rebuild strength, confidence and independence. You will deliver evidence based exercise physiology care, support recovery after complex health events and work closely with a multidisciplinary team to achieve safe discharge and hospital avoidance.

Assess patient needs and tailor exercise therapy plans

Deliver evidence based programs in home settings

Collaborate with nurses, doctors and allied health teams

Support safe discharge and hospital avoidance strategies

Guide assistants and students and contribute to service improvement
