To work within a specified service area, to provide the office/telephone liaison between clients, care employees, Case Managers and families to ensure that all client care and service requirements/issues are acted on, that all service requests are scheduled, that all actions are communicated to the relevant personnel and that all services are correctly verified when completed. Supporting the growth of Calvary Home Care business by overseeing the CHSP programme with in taking, assessments and reviews of new and current clients.

Your main duties will include: Intake CHSP approved clients. Annual reviews over the phone and assessments with CHSP clients. Work within KPI and expected time frames. Rostering and scheduling to ensure compliance with EA requirements. Answering and triaging phone calls. Some travel may be required

About you: Must have a willingness to work as a team. Initiative and motivation to learn new skills. Hold a current drivers license. Have previous experience, knowledge with rostering, scheduling, CHSP program funding or Community Home Care. Friendly communication skills to answer calls and deal with challenging conversations.
