Care Partner—Home Care

Parkview Community Care is a progressive in-home aged care provider that enables older people to stay home for as long as they are able.

Our business is growing, and we are looking for a skilled and ambitious person to be our aged care in-home care partner to liaise with our clients and staff, coordinating in-home care to enable clients to live safely and comfortably at home. You will have the ability to develop relationships with our elderly clients from the very first phone call and really make a difference in their lives.

The position istime, office-based role located in Moonta SA Company vehicles are available for client visits. Salary commensurate with experience plus superannuation plus salary packaging available.

You will be responsible for:

Assist with client goal setting and promote holisticbeing

Monitor client progress and satisfaction through regular engagement

Conducting assessments, developing support plans, and implementing clinical and care interventions

Ensure accurate documentation and compliance with aged care regulations

Competent IT skills, e.g. Outlook and Word (we will provide full training in our own database)

Strong customer service skills, friendly telephone manner and highly organised with excellent verbal and written communication skills

Superior time management and attention to detail

The ability to change direction and be flexible due to priority changes

What you will bring:

Registration as an EN or Certificate IV in Community Services/Ageing or similar equivalent qualification

Experience in the aged and/or disability sector, preferably in a Care Coordinator/Case Manager role

Budget management skills

Current unrestricted driver’s licence

Satisfactory National Police Clearance (or willingness to obtain)
