Two contract full time positions - Responsible for the coordination of the Passport Program within York Region, which focuses on adults with a developmental disability seeking community participation supports and/or skills development and respite supports.
Responsible for waitlist management, acting as a resource to eligible adults and their families around community options and administration of funding.
Requirement:
 B.S.W. or equivalent
 Minimum three years related experience
 Experience working with individuals who have developmental disabilities is essential
 Language skills and an understanding of cultural diversity are considered assets
 Familiarity with the local service system and its providers is required
 Second Language is an asset
 A valid G driver’s license, business use insurance and regular use of a reliable vehicle.
